• Family & Medical Leave Process – Sun Life 06/13/2022

I noticed today (06/28/2023) that my employer has updated an HR Compliance Document. I hope my condition has led to an improvement in the experience for anyone else suffering from a disease with a fatal prognosis.

Hi <Vice President of Customer Services>

I hope you had a great Holiday. Mine was filled with Love, Family, and Respect (plus breath-taking fireworks) at my home on the Pacific Coast of Washington state. Today as I’m cleaning up after several young nieces and nephews, I am thinking about those who remain to handle the “aftermath” and the respect they are afforded (specifically you and <direct Manager> in my absence).

It is in this tone that I apologize to you for being unavailable as a full-time resource for <the company> these past months and hope you know that I’d prefer to be working and enjoying employment! I also highlight the respect that I need for my prior contributions to HealthSparq success, in the words of <direct Manager> this past March – “Joseph, this promotion is for all the good work you have ALREADY done”.

Now, I have benefited greatly from being associated with <the company> and consider myself super-lucky with a continued position after the “mass-layoffs” earlier this year, and yet I am swallowing any pride (because how blessed I already am) to ask for even more!

My approval letter for Long-Term Disability benefit has been approved by <insurance policy holding company>. The first date/time was unable to return to work was the afternoon of January 5, 2023. At this date and time, my 2023 promotion salary was not reflected, and I’m asking if you would make that promotion date retroactive to January1.

As a fatal prognosis, Glioblastoma Multiforme statistics show there is a 95% chance I will not be alive in five years. So in summary :

How is <the company> doing financially? I’m hoping that after the <previously acquired company> position removals, <the company> can afford a few extra bucks to send my way! LMK? J.Ammons
